Wheeling Health Right
One year after the launch of HeWV at Wheeling Health Right, the clinic is the envy of its counterparts nationwide, according to Kathie Brown, executive director. “HEALTHeWV has put us on the cutting edge of health care for the uninsured,” Brown says. “Without it, we never could have afforded electronic medical records. It has allowed us to improve our patients’ health outcomes, and to collect the statistics needed to back up that claim. Free clinics across the country are quite envious of our EMR. ”Wheeling Health Right was the pilot site for HeWV, going live in August 2006. The clinic serves 1,000 patients per month in West Virginia’s northern panhandle. “Continuity of care has improved,” says Peggy Ferguson, a nurse practitioner at Wheeling Health Right. “We now have faster access to charts and notes from prior patient visits, which makes it easier to prepare for telephone calls with patients, to communicate with other members of the health care team, and to see and compare changes in test results.” HEALTHeWV has the support of Sen. Robert C. Byrd, who has worked for years to combat the high rates of heart disease, cancer and stroke among West Virginians. “Because our state is predominantly rural, many of our citizens who suffer from chronic disease conditions lack access to the best available health care,” Byrd says. “I fully support the HEALTHeWV program because it specifically targets improving health care for rural patients and gives rural physicians access to the same medical information as their urban counterparts.”

Implementing an EMR is an opportunity and a challenge. It is an opportunity to revise processes to improve care and it is a challenge because the staff must learn a new way of recording and managing information. HeWV organizes and presents information to medical providers in a way that promotes efficient clinical decision making. It also provides the opportunity for expanding the role of nurses and medical assistants in collecting and recording clinical data. However, it does require significant management attention to process design, training, and creating opportunities for discussion and problem solving.

Mercer Health Right implemented HeWV in November 2007. I found that the NTTC staff did an excellent job evaluating the state of the clinic’s computer readiness and recommending the components necessary for successful adoption of the HEALTHeWV system. Training was completed on a schedule at our convenience and tailored to our clinic personnel. NTTC support during our “go-live” week was excellent and although the expectation was to process e-charts with a few patients a day, we accomplished documenting our complete day’s patients in e-charts from the first day. The support manuals were well written and answer many of the questions that occur during startup. NTTC telephone and help desk support have also performed superbly. Each week we become better versed in the intricacies of the system and realize the efficiencies provided through the use of an EHR. Our clinic could not afford to divert funds to purchase a comparable commercial system. We have a new nurse practitioner who has been working in a medium sized physician practice with an EMR and she finds HEALTHeWV to be much more user friendly. We are performing successfully due to the training, staff support, and software from HEALTHeWV.

Cabin Creek Health Systems
“Now that we’re this far down the electronic pathway it would be difficult to imagine our future without an electronic health record,” says Sandra Mitchell, Operations Director for Cabin Creek Health Systems (CCHS). Cabin Creek implemented HEALTHeWV in its four primary care sites in Kanawha County in 2007. CCHS currently provides care to some 15,000 patients, resulting in approximately 45,000 visits annually. CCHS’s original clinic site is Cabin Creek Health Center in Dawes, W.Va. CCHS now includes three additional sites, along with a full-service pharmacy and laboratory. A fifth school-based site is slated to open in the fall of 2008. HEALTHeWV is a vital part of the long term strategic plan to improve care in these communities, according to Mitchell. “We consider electronic charting to be an essential tool in delivering cost-effective, quality care,” she says. A major reason for CCHS’s adoption of HeWV was the shortage of space for the expanding number of patient charts. Mitchell explains, “Saying ‘no’ to new patients wasn’t a solution consistent with our mission. An electronic health record will enable us to continue to say ‘yes’ to new patients in our communities.” In addition to solving space issues, HEALTHeWV helped CCHS with user constraints. At any point in time, a patient’s chart could be between provider, nurse, medical assistant, referral coordinator and prescription coordinator, resulting in time wasted in “chart chasing.” The proficient use of HEALTHeWV’s electronic health record eliminates many of the inefficiencies and frustrations associated with chasing paper charts. In providing quality care, today’s providers need efficient access to information and the ability to measure and track outcomes. HEALTHeWV provides valuable functions such as the diabetes and coumadin registries, which allow chronic disease patients’ individual progress to be monitored. Electronic health records are critical to collecting and evaluating outcome measures in a cost-effective manner. “Beginning with the 2008 Uniform Data Survey, required of all federal grantees of the BPH, we’re required to report outcome measures. HEALTHeWV will allow us to report this data far more efficiently,” Mitchell says. She recalls that after enduring the initial conversion frustrations, one provider commented, “I’d rather see patients now with HEALTHeWV than with a paper chart.” Mitchell credits Craig Robinson’s innovative leadership with making an electronic health record a priority for CCHS; and physician champion, Dr. Chad Turner, with success in implementing HEALTHeWV. She also commends CCHS’s providers and support staff for their hard work, which is critical to the success of the project. “Regardless of planning or training efforts, it still comes down to the diligence of providers and staff in making the project successful,” Mitchell says.

WCHSA-Ripley Family Medicine
Ripley Family Medicine, a subdivision of Wirt County Health Service Association (WCHSA), in working with the Coplin Clinic, River Valley Health and Wellness Center as well as the School Based Health Clinic at Ripley High School, has been 99% electronic since it opened September 3, 2007. I have had a lot of success with HEALTHeWV. Electronic health records allow me to track my patients’ progress from the first encounter through subsequent visits. The only charts we have in the office are from transferring patients.With continuous updates to the system, it becomes easier to use and more user-friendly. It is nice to be able to customize the tabs in the progress notes to make the note flow with the patient encounter. The only problems my office staff and I have had are not due to the application itself, but to the down time and speed capabilities of the internet service provider. One of the most beneficial aspects of the program is knowing when I am out of the office on maternity leave, my support staff will have access to my notes. If they have any questions, I will be able to access patient notes from home and provide feedback. I believe that the program makes the office run more efficiently because staff does not have to pull charts to check on medication refills, treatment plans, and questions about referrals and lab results. I can honestly say that I am pleased with the way that HEALTHeWV has helped with the continuing success of my office, Ripley Family Medicine.

Milan Puskar Health Right
Milan Puskar Health Right (MPHR) is a free clinic founded on the premise that everyone has a right to health care regardless of their insurance status or ability to pay. MPHR is celebrating 25 years of service to the community in 2009. Our patients have low incomes and no insurance. We serve patients in nine counties in North Central West Virginia. MPHR provides primary care, medications, individual and family counseling, referrals to some specialty care, HIV testing and community-based dental care at no cost to qualified patients. About four years ago, staff at MPHR heard rumblings about HEALTHeWV, a new electronic medical records system that could enhance care in clinics like ours. At the time, I didn’t realize I would become the director and oversee the transition to HEALTHeWV. Getting the free clinic up and running had its challenges. It took a huge community effort to help us purchase a new server, computers for exam rooms, and scanning capabilities. Morgantown, surrounding communities, and especially the McQuain Charitable Trust have helped make e-records a reality for us. The staff at the National Technology Transfer Center is friendly, helpful and was especially patient with us as we went online for the first time. Change is hard for everyone and for employees and volunteers (many of whom didn’t have much experience with computers or EMR) it has been a journey. We have been impressed by their willingness to deal with the frustrations that come along with learning new technology. Recently, MPHR established satellite clinics in Kingwood and Fairmont, W.Va. The HEALTHeWV system makes it possible to see patients at new locations and treat them in their local communities. Without HEALTHeWV, I have no doubt we would still be in the development stages for both of these new satellite clinics. Most importantly, HEALTHeWV allows us to improve the quality of our health care. Our medical staff and counselors are able to communicate quickly and patient information is readily available. We are especially excited about the diabetes registry within the program. Chart reviews in the past have been long and tedious, making it harder to determine outcomes for patients with diabetes. Now we have that information at our finger tips. Moreover, we can use the registry report to see where we need to focus and what educational areas need addressed.

Brooke County Health Department
The Brooke County Health Department is very excited to be the first health department in West Virginia to implement HEALTHeWV. Although other agencies such as hospitals have used electronic medical record reporting for many years, small clinics like health departments simply do not have the funds or resources to accomplish this. HEALTHeWV has changed this situation by providing small non-profits with not only the ability to participate in electronic record keeping but also with a comprehensive disease management program for the medically disadvantaged clients it serves. When I was first contacted by the National Technology Transfer Center (NTTC) at Wheeling Jesuit University about being a pilot site for HEALTHeWV among health departments, I wanted this agency to participate. As the director, I am always looking for ways to manage our operations in the most efficient way possible but had my doubts about whether or not my staff could make the transition. Some staff members were not comfortable using a computer at all and conducting a patient visit using technology seemed incomprehensible to them. However, the HEALTHeWV team worked one on one with the staff and slowly and carefully guided them through the process. Staff began to see the program as doable and also saw the positive outcomes; such as better patient management, improved documentation, and more time to spend with clients.
